Hazardous works management

1. Scope of Application

1.1. This standard sets out requirements for the organization and performance of the hazardous work carried out by employees of OOO “Continental Kaluga” structural divisions.

1.2. The requirements of the standard apply for all production structural divisions of the Company.


2. Referenced Codes and Standards

2.1. This standard is developed on the basis of the requirements containing in the following regulations, local standards and contains references to them:

2.1.1. The Federal Law No. 197-ФЗ dd. 30.12.01 "Labor code of the Russian Federation" (as amended on 09.05.05).

2.1.2. ПОТ (Labor Protection Rules) P M - 016. Interindustry rules on labor protection in operation of electricity generating equipment.

2.1.3. ПОТ P M-012-2000. Interindustry rules on labor protection for works at height.

2.1.4. The Federal Law No. 123-ФЗ dd. 22.08.2008 "Technical regulations on fire safety requirements"

2.1.5 ПБ (Safety Regulations) 10-382-00, Regulations on Installation and Safe Operation of Cargo Cranes.

2.1.6. ПБ 10-573-03, Regulations on Installation and Safe Operation of Steam and Hot Water Pipelines.

3. Terms, Definitions and Abbreviated Terms

3.1. The following terms with the corresponding definitions are used in the standard:

3.1.1. Hazardous work - work that is associated with dangerous and (or) harmful production factors affecting a worker, and requires appropriate measures of ensuring safe working conditions.

3.1.2. Dangerous factor of production - a production factor that, when affecting a worker, causes personal injury.

3.1.3. Harmful factor of production - a production factor that, when affecting a worker, causes contraction of a disease.

3.1.4. Working conditions - set of factors of the production environment and the labor process affecting working capacity and health of a worker.

3.1.5. Safe working conditions - working conditions under which the effect of harmful and / or dangerous production factors on workers is excluded or levels of their effect do not exceed the established norms.

3.1.6. Management of dangerous and harmful production factors - issuance of work permit for hazardous work and its performance according to it.

3.1.7. Joint works - works performed at the same time, at the same site, and at the same height level by two and more foremen.

3.1.8. Combined works - works performed at the same time, at the same site, at two or more height levels by one or more foremen.

3.1.9. Work permit - a written work task issued on the special blank of the established form and specifying the scope of work, work area, work start / finish time, safe working conditions, crew members and the employees responsible for work safety.

3.1.10. Works Organization Plan (WOP) - an organizational and administrative document developed by the Company’s structural division specialists to set out technical and organizational measures ensuring safe working conditions and requirements of industrial safety during preparation, major repair and maintenance of main equipment.

3.1.11. Work permit issuer – a structural division employee authorized by the order of the structural division head to carry out administrative functions on organization and performance of the hazardous work according to the work permit.

3.1.12. Work manager - a production structural division employee determined by the work permit issuer and authorized by the order of the structural division head to carry out administrative functions on management of hazardous work performance according to the work permit.

3.1.13. Work clearing person - a production structural division employee authorized by the order of the structural division head to carry out, prior to hazardous work performance, organizational and technical measures ensuring safe working conditions for work performers under the work permit and also to carry out control and supervision over the implementation of these measures and to give out the foreman the permission to perform hazardous work by making an entry into appropriate section of the work permit.

3.1.14. Foreman - a production structural division employee authorized by the order of the structural division head to perform hazardous work according to the work permit by direct management of such work performers and appointment of the supervisor.

3.1.15. Work performer - a worker under employment contract, having the profession and the qualification appropriate for the nature of hazardous work assigned, and performing such work according to the work permit.

3.1.16. Supervisor - a member of the work performers’ crew, appointed, if necessary, by the foreman to carry out at the workplace during the whole shift the supervision over the compliance of work conditions with the measures specified in the work permit.

3.1.17. Hot works - works at temporary locations involving heating with flame; gas and arc welding operations, gas and electrocutting operations, gasoline and kerosene cutting operations, soldering works, metal cutting by mechanized tool.

3.1.18. Hazardous gas works - works with presence or possibility of explosive or harmful vapors and gases emitting into a working zone in quantities capable to cause explosion, fire, harmful effects on a person, and also works with insufficient content of oxygen (under 20% by volume).

3.2. The following abbreviations are used in the standard:

3.2.1. The Company – OOO “Continental Kaluga”

3.2.2. WOP - works organization plan


4. General provisions

4.1. Hazardous works performed at the production structural division of the Company are carried out on the basis of the list of such works (appendix 1) compiled for specific structural division of the Company.

The following types of works must be referred as the hazardous works:

- Hazardous gas works;

- Hot works;

- The repair, construction and installation works performed by employees of contractor organizations or of one production structural division of the Company on the territory of the other production structural division or under operating production conditions;

- Joint works;

- Combined works;

- Steeplejack works and works at the height of more than 1.5 m without scaffolding or within 2 m from unguarded elevations exceeding 1.5 m;

- Any works in closed venues and limited spaces (in wells, tunnels, tanks) etc.;

- Works on the roofs of buildings and structures (roof repair, snow or dust cleaning, etc.);

- Installation / dismantling, repair and maintenance of the compressed air, hot water and steam pipelines;

- Works involving use of pyrotechnic tools, dowel-drivers;

- Works on elimination of incidents and accidents consequences;

- Earth works in underground service lines zone (pipelines, cables and so forth);

- Works in electricity generating plants without de-energizing or with partial de-energizing of the voltage exceeding 42 V.

- Operation of two and more cargo cranes in one area, as well as operation of a crane within 30 m from the high voltage line;

- Repair and adjustment using sources of ionizing radiation.

- Repair or any other works on crane runways and operating bridge crane passages.

- Opening, repair and hydraulic test of pressure vessels.

- Other works in areas subjected to dangerous and harmful production factors.

4.2. The list of the hazardous works (appendix 1) shall be compiled by the specialists of the structural division (site engineers, process engineers), signed by the technical manager of the structural division, agreed with Safety and Environmental Affairs, and approved by the chief engineer.

The said list shall be revised annually, by no later than February 1, in order to add new types of the hazardous works, planned for the current year, and to remove the works which are not needed anymore.

4.3. Heads and specialists of structural divisions authorized to issue work permits may recognize the works, not included in the said list, as hazardous works. In doing so, they have to identify dangerous and harmful production factors associated with the work to be performed, and to organize performance of such works according to the standard.


5. Requirements for the organization of the hazardous works

5.1. Requirements for employees

5.1.1. The employees of the working professions involved in performance of hazardous works, must:

- be professionally trained and qualified for works performed;

- undergo health check and have no medical contraindications for performance of works under their main and concurrent professions;

- undergo assessment of knowledge of labor safety requirements for the main and concurrent professions;

- have permit for unsupervised work under the main and concurrent professions;

- undergo pre-job briefing according to the work permit;

- observe the instructions received at the pre-job briefing.

5.2. Requirements for ensuring safe working conditions at the hazardous works

organization

5.2.1. The main requirement for ensuring safe working conditions during the organization of the hazardous works is to identify dangerous and harmful factors affecting the workers during the forthcoming work performance and to manage them by means of the work permit of the appropriate form.

5.2.2. The hazardous works, carried out by employees of structural divisions of the Company and contractors, shall be organized by means of work permit issuance according to relevant rules, for example, ПОТ Р М - 016 or ПБ, or, unless provided otherwise, according to appendix 3.

5.2.3. The hazardous works shall be carried out in strict accordance with the measures ensuring safe working conditions provided by the work permit.


6. Requirements for the hazardous works preparation according to the work permit

6.1. The employees ensuring safe working conditions for hazardous works performed under work permit are:

- work permit issuer;

- work manager;

- work clearing person;

- foreman;

- supervisor;

- work performer.

6.2. Responsibilities of the employees specified in paragraph 6.1 (except the work performer) are defined by the head of the production structural division by approval of the list of positions (appendix 2), which vest the employees, assigned to them, with corresponding responsibilities. Responsibilities of the work performer are defined by the work permit issuer or the foreman by giving job assignment, per-job briefing, and filling in the section 4 of the work permit.

6.3. Copies of the list of employees vested with the duties of hazardous works organization and performance shall be submitted to the heads of divisions who shall familiarize their subordinates, vested with such duties, with it via their signatures.

6.4. The work permit shall be issued by the work permit issuer in two copies and registered in the work permits log book (appendix 4).

6.4.1. One copy of the work permit shall be given out to the foreman, who confirms the receipt with his or her signature in the above mentioned log book, and kept directly at the work site by the foreman or the supervisor.

6.4.2. The other copy of the work permit shall be kept by the work permit issuer.

6.5. The copy of the work permit issued to the foreman shall be closed after completion of the work. The work permit is closed with signatures of the foreman and the work manager (if appointed).

6.6. The closed work permit shall be returned by the foreman to the work permit issuer. The work permit issuer must keep both copies of the work permit for 30 days from the date of closing.

6.7. If the foreman or the supervisor loses the work permit, the works shall be suspended. A new work permit shall be issued for remaining works, and work performers shall be cleared to work again.


7. Organization of the hazardous works in electricity generating plants

7.1. The hazardous works in electricity generating plants shall be performed according to the safety requirements set by the Interindustry rules on labor protection in operation of electricity generating equipment ПОТ P M - 016.

7.2. Permit for works in electricity generating plants shall be drawn up according to the form established by ПОТ РМ - 016-2001 is used. Licensed software products, similar to "Naryad - Minuta", may be used to fill in the work permit.

7.3. Permits for works in general factory electricity generating plants shall be issued by the leading power engineer or the person performing his duties.

7.4. Permits for works in electricity generating plants of production sites shall be issued by employees in charge of electrical facilities of the production site, appointed by the order of the chief engineer.

7.5. Permits for works in electricity generating plants shall be accounted and kept by:

- the leading power engineer – for general factory electricity generating plants

- employees in charge of electrical facilities of production sites – for electricity generating plants of production sites.


8. Hazardous gas works organization

8.1. Hazardous gas works are regulated by ПБ 12-529-03, Safety Rules for Gas Distribution and Gas Consumption Systems and shall be performed according to the work permit described herein (appendix 5).

8.2. Before the works and also during long (more than 1 hour) breaks, the mandatory air composition analysis shall be made, and the results shall be indicated in the work permit. If the air analysis is not possible, works shall be carried out with self-contained breathing apparatuses.

8.3. Routinely performed works, carried out by the same staff, may be performed without work permit, provided the operating procedures for performance of such works are developed and approved by the chief engineer. The list of the works, which can be performed without work permit, shall be compiled and approved by the chief engineer.

8.4. Closed work permits shall be kept at the production division for one year.


9. Responsibilities of the employees ensuring safe working conditions for the works performed under work permit

9.1. Responsibilities of the work permit issuer.

9.1.1. The head or the specialist of the structural division, whose position is specified in the list of positions approved by the head of the structural unit (appendix 3), are authorized to issue work permit and to give it to the foreman.

9.1.2. When issuing a work permit, the work permits issuer shall:

- identify the dangerous and harmful production factors associated with the works to be performed;

- identify the necessary measures ensuring safe working conditions for the work performers;

- appoint the foreman and, in case of complex works (for example, joint or combined works), the work manager and the work clearing person;

- conduct pre-job briefing on the safety measures provided by the work permit for the work manager, the work clearing person and the foreman.

9.1.3. If necessary, the work permit issuer also performs duties of the work manager.

9.1.4. The work permit issuer is responsible for completeness of the measures ensuring safe working conditions specified in the work permit.

9.2. Responsibilities of the work manager

9.2.1. The work manager is appointed by the work permit issuer in case joint and / or combined works are necessary. The head or the specialist of the structural division, whose position is specified in the list of positions approved by the head of the structural unit (appendix 2), can be appointed as the work manager.

9.2.2. The work manager manages the performance of hazardous works by coordinating actions of the work clearing person(s) and the foremen. Also the work manager shall:

- check whether every dangerous and harmful production factors have been taken into account in the work permit;

- determine whether the measures ensuring the safe working conditions specified in the works permit are adequate;

- if necessary, define additional measures ensuring safe working conditions and add the corresponding instructions in the work permit;

- supervise the implementation of the specified measures;

- if necessary also perform duties of the work manager and the work clearing person;

- supervise the implementation of the primary and additional measures ensuring safe working conditions for work performers;

- stop the works in any event of threat to life and health of work performers or if works are performed in conditions other than those defined by the work permit;

- be responsible for the adequacy of primary and additional measures ensuring safe working conditions.

9.3. Responsibilities of the work clearing person

9.3.1. The head or the specialist of the structural division, whose position is specified in the list of positions approved by the head of the structural unit (appendix 2) and who is in charge of the site of the hazardous works to be performed, can be appointed as the work clearing person.

9.3.2. The work clearing person is appointed by the work permit issuer if complex preparatory works, for example, joint or combined works, are necessary or when the shutdown of the working equipment requires disconnection or switching of power supply systems, etc.

The work permit issuer may vest the foreman with the duties of the work clearing person.

9.3.3. The work clearing person shall:

- ensure the implementation of the necessary measures provided by the work permit to ensure safe working conditions during preparation to hazardous works, before the performance of such works;

- inform the foreman on the implemented measures ensuring safe working conditions and the measures to be implemented while performing works according to the work permit;

- clear foreman to begin work performance by making clearing record in the work permit;

- supervise the observance of the measures provided by the work permit;

- suspend the works, withdraw the work permit from the foreman or the supervisor and report the work permit issuer and the work manager on all cases of emergence of threat to life and health of work performers or if works are performed in conditions other than those defined by the work permit;

- be responsible for implementation of the measures ensuring safe working conditions, specified in the section "Before the works" of the work permit.

9.4. Responsibilities of the foreman

9.4.1. The head or the specialist of the structural division, whose position is specified in the list of positions approved by the head of the structural unit (appendix 2) and who is in charge of the site of the hazardous works to be performed, can be appointed as the foreman.

9.4.2. The foreman is appointed by the work permit issuer, usually, from among heads or specialists of structural division being in charge of subordinates who can be entrusted with the performance of the hazardous works.

9.4.3. The foreman shall:

- determine the crew of work performers and their qualification;

- brief work performers on the necessary measures ensuring safe working conditions;

- during the hazardous works performance, implement measures ensuring safe working conditions according to the work permit;

- by order of the work permit issuer, concurrently perform duties of the work clearing person;

- personally supervise the implementation of the measures ensuring safe working conditions, receive the work clearing record from the work clearing person (if work clearing person is not appointed, make work clearing record in the work permit);

- appoint the supervisor if complex works (for example, joint or combined works) are necessary;

- accept the works performed under the work permit from the shiftman, personally check the working conditions, make the clearing record in the work permit or receive work clearing record from the work clearing person;

- give clearance to resume the works after breaks during the shift (lunch break, break related to work conditions);

- renew the work permit after the break for longer than one shift, and in doing so check the implementation of measures ensuring work performers’ safety, make the clearing record in the work permit or receive clearance to resume works from the work clearing person;

- supervise work performance and implementation of the measures ensuring safe working conditions and belonging to work performers’ responsibilities;

- suspend the works in any event of threat to life and health of work performers at his own discretion or by order of the work clearing person or the work manager (for example, on their order remove work performers to a safe place during equipment motoring and test run);

- resume the works after having personally checked the implementation of the measures ensuring safe working conditions and made work clearing record in the work permit or after having received clearance from the work clearing person or the work manager;

- be responsible for worker’s qualification adequacy for the nature of the works performed and for the implementation of the measures provided by the work permit and ensuring safe working conditions during and after work performance.

9.4.4. Through an exception, the work permit issuer may appoint a crew member having appropriate qualification as the foreman. In doing so, the works permit issuer must determine the crew of work performers and brief them, and concurrent duties of the foreman and the work clearing person shall not be allowed.

9.5. Responsibilities of the supervisor

9.5.1. The supervisor is appointed by the work permit issuer or the foreman from among the most qualified work performers if difficult to manage works (for example, joint or combined works) are necessary.

9.5.2. The supervisor shall:

- receive pre-job briefing from the work permit issuer or the foreman and confirm it with his signature in the work permit;

- personally supervise the performance of works during the work shift and implementation of the measures ensuring safe working conditions and belonging to work performers’ responsibilities;

- suspend the works in any event of threat to life and health of work performers and notify the foreman about it at his own discretion or by order of the work clearing person or the work manager;

- resume works on the instruction of the work clearing person or the foreman;

- be responsible for the implementation of the measures ensuring safe working conditions, belonging to work performers’ responsibilities and defined in the works permit, during the performance of works.

9.6. Responsibilities of the work performer (crew member)

9.6.1. Work performers are determined by the work permit issuer or the foreman during work permit issuance.

9.6.2. Work performer shall:

- properly apply and use overalls and personal protection equipment during works;

- perform the works assigned in strict accordance with the work permit;

- observe the requirements of the briefing received from the foreman, of the guidelines setting out his or her duties (technology, operation, job, and labor safety guidelines), instructions of the foreman and the supervisor;

- observe safety requirements provided by the work permit;

- stop performance of works by order of the foreman or the supervisor;

- be released from work performer’s responsibilities when appointed as the supervisor;

- be responsible for implementation of the measures ensuring safe working conditions, defined by the work permit and belonging to his or her responsibilities


10. Requirements for development and application of the work organization plan (WOP)

10.1. WOP is formed by specialists of structural division or contractor companies according to the appendix 5 for capital and routine repair works, installation / dismantling of the major equipment, installation and construction works.

All other types of repair works do not require WOP.

10.2. Before the performance of works under WOP, all work managers and foremen must be familiarized with its contents.


11. Requirements for ensuring safe working conditions during organization of works on elimination of incident and accident consequences

11.1. Works on elimination of incident and accident consequences shall be carried out in compliance with the following requirements:

11.1.1. For elimination of incident and / or accident consequences, the emergency recovery operations manager (usually, specialist in charge of operation of the site, where the emergency recovery operations are to be carried out) shall be appointed by the Company order (decree) or the decree of the head of the production structural division.

11.1.2. The emergency recovery operations manager is in charge of the employees of the production structural division and work managers of the contractor companies involved in emergency recovery operations.

11.1.3. The emergency recovery operations manager shall:

- have the right to manage emergency recovery operations at his or her sole discretion;

- at his or her sole discretion or in coordination with work managers of contractor

companies, take technical or organizational decisions on implementation of emergency recovery operations;

- at his or her sole discretion or in coordination with work managers of contractor companies, identify the measures necessary for ensuring safe working conditions for workers;

- give works orders, binding for all persons involved in elimination of incident and accident consequences.

11.1.4. After elimination of the dangerous and harmful production factors which have emerged due to incident or accident, and after elimination of threat of damage to the equipment, works on elimination of incident and accident consequences shall be carried out according to the requirements of section 5.2 hereof.


12. Requirements for the organization of works performed at structural production divisions of the Company by the employees of contractor companies

12.1. All types of works performed at structural divisions of the Company by employees of contractor companies are considered hazardous works and shall be organized according to the requirements of the "Management of Contractor Companies" procedure and the provisions hereof.


13. Liability

13.1. The employees shall be liable for non-fulfillment (improper fulfillment) of the requirements hereof according to the applicable legislation.
